RE: Special Event Liquor License - McHenry Downtown Business
Association
The McHenry Downtown Business Association is requesting a Special Event Liquor
License and waiver of all associated fees to serve champagne at its annual “Oscar
Party” to be held in the McHenry Theater on Green Street. All profits from the event
will be used for future downtown events and revitalization programs. The event is
scheduled for Sunday, February 29, 2004 from 2:00 PM to 7:00 PM. A copy of the
Special Event Liquor License Application is attached.

CONSENT AGENDA

RE: Requests for Business Retention Loan
Background. At the October 13, 2003 meeting, the City Council approved the
Business Retention Loan Program designed to assist those businesses affected by the
Illinois Route 120 reconstruction project in meeting their day-to-day operating
expenses. To date, the City has approved two loans (West Elm Laundry and Skyline
Foto Fast).
Analysis. Two applications have been received and are presented for your review:
Elm Street Medical and McHenry Health Foods, LLC.
The Business Retention Loan Committee has considered both applications and
unanimously recommends approval. Copies of the two loan applications are
attached for your review.
Recommendation. The business retention loan committee unanimously
recommends approval of the two loan applications - Elm Street Medical Center and
McHenry Health Foods, LLC in the amount of $7,500 for each loan.

RE: Municipal Water System Analytical Testing
RECOMMENDATION: To approve the attached three-year agreement with McHenry Water
Laboratory, Inc. not to exceed $27,594.00.
Background. The Safe Drinking Water Act provides the Illinois Environmental Protection Agency
(IEPA) with the responsibility of establishing drinking water standards in the State of Illinois.
Through the IEPA’s Drinking Water Standards Program, municipal water supplies must meet water
quality standards through routine testing. The results of the analytical tests are compiled and
distributed in an Annual Consumer Confidence Report. The city currently holds an agreement with
McHenry Analytical Laboratory, Inc to perform testing of its water supply and to prepare the report.
This agreement expires soon and as a result, staff solicited proposals for a three-year agreement for
testing services.
Analysis: Proposals were received from two companies, IEPA Community Water Supply
Testing Fund (CWSTF) and McHenry Analytical Laboratory, Inc. The three-year contract proposal
with CWSTF was $15,606 higher than McHenry Analytical Laboratory’s price of $27,594. In
addition, McHenry Analytical Laboratory has a successful track record with the city having
performed testing and lab work for the city for the past six-years.
Recommendation. To approve the attached three year agreement with McHenry Water Laboratory,
Inc. for analytical testing of the city’s water supply for an amount not to exceed $27,594.00.

RECOMMENDATION: To award contract for the construction of Water Supply Well No.
12 to Manusos General Contracting, Inc. not to exceed $229,000.00.
Background. At its January 5, 2004 meeting, the City Council authorized Baxter and Woodman
to solicit bids for the construction of a shallow well (No. 12) within the Shamrock Farms
Subdivision.
Analysis. Tuesday, February 3, 2004 the following bids for the Water Supply Well No. 12
project were received:

Baxter and Woodman tabulated and analyzed each of the bids and finds Manusos General
Contracting, Inc. to be in compliance with the bid specifications, the lowest responsible bidder, and
recommends approval of the contract
Recommendation. To award contract for Water Supply Well No. 12 Drilling, Pumping Equipment,
and Chemical Feed Building to Manusos General Contracting, Inc. not to exceed $229,000.00.
