The Consent Agenda for the April 19, 2004 City Council meeting consists of the

RE: Employee Assistance Program
Recommendation: To authorize an agreement with Advantage EAP Program (EAP)
for
Fiscal Year 2004/2005 not to exceed $5,655.
Since 1996, the city has contracted the services of the Employee Assistance Program.
This program, a division of Family Service & Community Mental Health Center,
provides both the employer and employee with confidential and professional
intervention in personal problems that may affect work performance. The program
helps to resolve these personal problems and restores troubled employees to their
maximum productivity.
In addition to counseling assistance and referrals, the EAP provides educational
materials, workshops and seminars. McHenry Family Services offers high quality
services and is located within the city limits.
The amount of the contract is based on the number of employees and is payable at
the beginning of each contract period in May and November. This fiscal year, the
amount of the annual contract is $5,655.
