Adjournment Geri A.Condon Posted and Mailed: March 8, 2006 RESOLUTION R-06-001 WHEREAS, Donald Doherty has submitted his resignation and has served as a founding and charter member of the McHenry Economic Development Commission at its inception September 29, 1981; and WHEREAS, he has given unselfishly of his time and talents for the betterment of the McHenry Community through volunteering on the Commission and acting as a pro-active liaison between the McHenry County Board's Transportation Committee and the McHenry Economic Development Commission, and has provided invaluable assistance in promoting the history, tourism and numerous other beneficial qualities the City of McHenry has to offer both 'commercial and industrial entities who are seeking relocation to a new environment; and WHEREAS, Don has demonstrated pursuit of economic wellbeing for the McHenry Area through his initiative in securing a hotel for the community, supporting the expansion of the industrial/commercial tax base with the annexation and development of McHenry County's first Corporate Business Park, being instrumental in securing the West McHenry Bypass, assisting in the EDC System Review Survey, and providing encouragement for existing business entities to aesthetically improve the entryways to the City and its commercial hubs; and WHEREAS, the City of McHenry in conjunction with the McHenry Economic Development Commission wish to recognize the efforts, commitment and drive exhibited by Donald Doherty and acknowledge the positive impact his efforts have had, and will continue to have, on the citizens of the McHenry Community. NOW, THEREFORE, BE IT RESOLVED that the McHenry City Council and the McHenry Economic Development Commission jointly go on record as recognizing Donald Doherty's 24 years of dedicated service and commitment to our community and express their appreciation and gratitude along with their best wishes for Don in his future endeavors. As Needed Checks Attachments REGULAR MEETING MARCH 6, 2006 The Regular Meeting of the McHenry City Council was called to order by Mayor Low at 7:30 P.M. on Monday, March 6, 2006 in the Council Chambers of the Municipal Center. At roll call the following Aldermen were present: Santi, Glab, Schaefer, Wimmer, Peterson, and Condon. Absent: Murgatroyd. City Staff in attendance were: City Administrator Maxeiner, Assistant City Administrator Martin, City Attorney McArdle, Chief of Police O'Meara. Director of Community Development Napolitano, Director of Public Works Marcinko, Community Relations Management Assistant Hobson, and Director of Finance Shields, Director of Parks and Recreation Merkel. Absent: City Clerk Jones. TEN MINUTE PUBLIC INPUT SESSION Vince Nett of 405 North River Road addressed Council regarding police misconduct. Mr. Nett stated he was assaulted in a local restaurant. Mr. Nett advised the reporting police officer he wished to file a complaint. He summarized the incident and indicated he wished to make a complaint regarding the way he was treated by security personnel at Northern Illinois Medical Center and by the McHenry City Police Department Officers. To date, no formal action has been taken. CONSENT AGENDA Alderman Peterson requested Consent Agenda Item A(2) Approval of City Council Minutes of the February 13, 2006 regularly scheduled meeting be removed for correction prior to approval. Alderman Peterson stated his reference was to "not being comfortable without a judge's decision regarding Maryetta Lane ownershir. Motion by Peterson, seconded by Condon, to approve the Minutes of the February 13,2006 regularly scheduled Council Meeting as amended. Voting Aye: Santi, Glab, Schaefer, Wimmer, Peterson, Condon. REQUEST FROM KIWANIS CLUB OF MCHENRY TO BLOCK CERTAIN PORTION OF GREEN STREET AND TO DIRECT STAFF TO ASSIST IN THE FOURTH ANNUAL SOAP BOX DERBY EVENT ON SATURDAY. JUNE 17, 2006 Management Assistant for Community Relations Hobson stated the McHenry K'iwanis Club is seeking Council support of their Fourth Annual Soap Box Derby Event which involves the intermittent closure of a portion of Green Street. The Kiwanis are also seeking Staff support and assistance for the provision of police officers, signage, barricades, bleachers, refuse disposal containers, reviewing stand, and public address system for the event. Management Assistant Hobson stated this year's event will not include a car show. However, an additional class of soap box derby cars has been included which should level out the timing of the laps being run in the event. Motion by Santi, seconded by Schaefer, to accept Staffs recommendation to: • approve the request to block off certain portions of Green Street on an intermittent basis for the Kiwanis Fourth Annual Soap Box Derby to be held on June 17, 2006; • approve a practice event to be held on June 3, 2006; and • direct Staff to provide assistance for the event including, police officers, signage, barricades, bleachers, refuse disposal containers, reviewing stand, and public address system. Voting Aye: Santi, Glab, Schaefer, Wimmer, Peterson, Condon. Voting Nay: None. Absent: Murgatroyd. Motion carried. REQUEST FOR CLASS F(1) LIQUOR LICENSE FROM CORKSCREW POINTE, INC. DIBIA CORKSCREW POINTE, 1402 NORTH RIVERSIDE DRIVE: AND APPROVAL OF AN ORDINANCE INCREASING THE NUMBER OF CLASS F(1) LIQUOR LICENSES FROM THREE (3) TO FOUR (4) Mayor Low stated she has met with the applicants for Corkscrew Pointe who are March 6, 2006 Page 3 requesting a Class F(1) Liquor License for their new business located at 1402 North Riverside Drive. All necessary paperwork and fees have been paid. The applicants have submitted to required fingerprinting. Mayor Low stated she recommends approval of the Liquor License as well as an ordinance providing for an increase in Class F(1) Liquor Licenses from three (3) to four(4). Motion by Condon, seconded by Santi, to accept the Mayor's recommendation to: e approve a Class F(1) Liquor License for Corkscrew Pointe for their business located at 1402 North Riverside Drive pending results of the required fingerprinting and that all employees involved in the sale/service of alcoholic beverages attend BASSETT training class; and e approve an ordinance increasing the number of Class F(1) Liquor Licenses from three (3) to four (4). Voting Aye: Santi, Gab, Schaefer, Wimmer, Peterson, Condon. Voting Nay: None. Absent: Murgatroyd. Motion carried. REQUEST FOR A CONDITIONAL USE PERMIT FOR A DRIVE-THRU FACILITY AT 2182-2210 WEST ROUTE 120— MD DEVELOPMENT Director of Community Development Napolitano reminded Council this matter was last considered by Council on February 20, 2006 at its regularly scheduled meeting. At that time, Staff was directed to draft a specific list of approved uses for the requested drive-thru facility at 2182-2210 West Route 120 requested by MD Development. Director Napolitano stated Staff believes allowable uses should include: e ice cream, donut, yogurt, candy, nut and popcorn shops; e dry cleaners, with no on-site processing; e coffee, tea and other non-alcoholic beverage shops; e video rental; e photo shops; and e small drive-thru restaurant. Director Napolitano noted Staff has determined a typical fast-food restaurant would not locate at this site as it is not large enough. Director Napolitano stated the applicant has concurred with Staffs recommendation and will agree to the specified uses for the requested conditional use permit. In response to Alderman Santi's inquiry, Applicant. Michael Baglio stated there would be only one window for the drive-thru. It would be used for both ordering and pickup. Stacking area would prohibit having two windows at the facility. Alderman Glab stated he was not comfortable approving a conditional use for a drive-thru facility when the specific use is unknown. Responding to an inquiry, Mr. Baglio stated he has no possible tenants at this time. A brief discussion followed regarding the expiration of the conditional use if not utilized for a period of one year or more. March 6, 2006 Page 4 Motion by Schaefer, seconded by Santi, to accept Staffs recommendation to pass an ordinance granting a conditional use permit for a drive-thru facility to MD Development for their project located at 2182-2210 West Route 120, limited to the following uses: • ice cream, donut,yogurt, candy, nut and popcorn shops; • dry cleaners, with no on-site processing; • coffee, tea and other non-alcoholic beverage shops; • video rental; • photo shops; and • small drive-thru restaurant. REQUEST TO WANE BID REQUIREMENTS FOR RIVERWALK PHASE I BOULDER AND BEDDING INSTALLATION AND ENTER INTO A CONTRACT WITH ALLIANCE CONTRACTORS FOR$84,363.50 Management Assistant Hobson reported the Riverwalk Phase I boulder and bedding installation in preparation for the seawall is time-sensitive as the Boone Creek water level is low at this time. The Fox Waterway Agency will open the locks causing the water elevation to rise sometime in April. Management Assistant Hobson noted City Engineers have sought three quotes for this project with the lowest being submitted by Alliance Contractors. Staff is seeking Council authorization to waive the bidding requirements and to accept the quote submitted by Alliance Contractors for this project in the amount of $84,363.50. In response to Alderman Peterson's inquiry, Management Assistant Hobson stated as Alliance Contractors is also repairing the seawall following the boulder and bedding installation, the project should be completed in two to three weeks. Alderman Peterson opined the cost for the project seems reasonable, particularly in light of the fact that three quotes were sought and Alliance Contractors came in with the lowest amount Alderman Condon expressed her pleasure in the Riverwalk Project coming to fruition after so many years in the planning. Alderman Santi concurred with Alderman Peterson regarding the proposed cost for this portion of the project. Responding to an inquiry, City Engineer Robert Wegener stated the river locks will re-open in the spring; however, the exact date is unknown and dependent upon the amount of rain sustained in the area. Motion by Schaefer, seconded by Santi, to accept Staffs recommendation to enter into a contract with Alliance Contractors for the Riverwalk Phase I boulder and bedding installation in the amount not-to-exceed $84,363.50. ADDENNDUM NO 1 TO ENGINEERING SERVICES AGREEMENT FOR WATER STORAGE TANK NO 4 CONSTRUCTION SERVICES IN THE AMOUNT OF $20,092 50— SMITH ENGINEERING SERVICES Director of Public Works Marcinko reported Staff is seeking authorization to approve an addendum to the Engineering Services Agreement with Smith Engineering for construction services regarding water storage tank number 4 in the amount of$20,092.50. Due to changes in the original contract and associated services, the addendum is being requested. Director Marcinko noted the City will recoup almost $19,000 from the monies expended by this addendum. Responding to Alderman Santi's inquiry, Director Marcinko noted this facility is the City's first one million gallon elevated water storage tank. He stated Tank No 3 located in Adams Commercial Centre has a 750-million gallon capacity. Motion by Wimmer, seconded by Peterson, to accept Staffs recommendation to approve Addendum No 1 to the Engineering Services Agreement for Water Storage Tank No 4 Construction Services with Smith Engineering Consultants in an amount not-to- exceed $20,092.50. Voting Aye: Santi, Glab, Schaefer, Wimmer, Peterson, Condon. Voting Nay: None. Absent: Murgatroyd. Motion carried. MAYOR— STATEMENT AND REPORT Mayor Low reported Mayor Ila Bauer of Round Lake Park passed away last evening. Mayor Low publicly expressed the City of McHenry's heartfelt sympathy to the family and community on their loss. Mayor Low noted Mayor Bauer initiated the CHARACTER COUNTS! Program in Round Lake Park which provided the foundation for the City of McHenry Coalition and she would be forever grateful for her leadership and friendship. COMMITTEE REPORTS Parks and Recreation Committee Chairman Alderman Peterson reported a Parks and Recreation Committee Meeting has been scheduled for Wednesday, March 8, 2006 at 7:00 p.m. in the Aldermen's Conference Room. Chairman Alderman Peterson also noted the Open House Kick-off for the Parks and Recreation Master Plan is scheduled for Thursday, March 9, 2006 from 6:30 p.m. until 9:30n p.m. in the Council Chambers. March 6, 2006 Page 6 STAFF REPORTS City Administrator Maxeiner reported the Police Department will conduct a live test of all emergency sirens tomorrow morning, Tuesday, March 7, 2006 at 10:00 a.m. City Administrator Maxeiner also reported the Riverwalk repairs and seawall construction will begin Tuesday morning, March 7, 2006. Finally, City Administrator Maxeiner reported extensive progress had been made with the federal mediator in discussions regarding Collective Bargaining and, consequently, the need for Executive Session to discuss Collective Bargaining this evening has been nullified. FUTURE AGENDA ITEMS Alderman Glab inquired as to the excavating currently underway south of Church of Holy Apostles and requested assurance it is being done with benefit of a permit being granted. Director Napolitano responded the only work being done on these premises is under the auspices of the Site Development Permit. Marcinko,Director of Public Works FOR: March 13, 2006 Regular City Council Meeting RE: Municipal Building Elevator ATTACMENTS: Otis Invoice On January 12, 2005 the Otis elevator in the municipal building was briefly out of service. Since the city contracts Otis to perform the annual inspection and maintenance, they were dispatched to diagnose the problem. Due to the nature of the problem and the City's responsibility to maintain accessibility for the public, this repair was declared an emergency action and Otis was authorized to make the necessary repairs. The repairs included the replacement of the internal computer system and the mechanism that controls the doors. This is the first major repair the elevator has needed since it's installation in 1991. Otis has submitted an invoice and is requesting payment for the emergency repair of the elevator. As the Council is aware, the IEPA has approved the operational needs report (ONR) and its recommendations the City submitted for the central wastewater treatment plant. Consequently, at the November 14, 2005 Regular City Council meeting, the City Council approved the engineering service agreement with Smith Engineering Consultants, Inc. (SEC) for the facilities planning area amendment. As part of the agreement, SEC provided the necessary engineering design services to implement the "band-aid"improvements to the central wastewater treatment plant outlined in the ONR. Analysis. SEC is finalizing the plans, specifications and bid documents for the improvements to the central waste water treatment plant and staff is requesting Council's authorization to advertise for bids. The engineers estimate for this project is $859,000.00. This project is funded through the Utility Improvements Fund. Recommendation. Staff seeks Council authorization to advertise for bids for the construction of the central wastewater treatment plant improvements. The new lot would be approximately 81'x 193' (15,209 sf.). Both would conform to the RS-2 bulk requirements. Staff Analysis Although Staff initially expressed concerns about the proposed RS-2 zoning for this site, the original recommendation was to approve the requested map amendment. Subsequent to the Planning and Zoning Commission meeting and after considerable discussion with the City Attorney, Staff is justified in denying the map amendment because the applicant does not meet all of the criteria for a zoning map amendment,specifically 1 and 4. Criterion one is compatibility with use or zoning of environs. The lots on the south side of Lincoln Road are generally larger than would be the newly created lot-if the zoning was approved. Also, criterion 4 is fiuthers public interest. We believe that this zoning does not and only furthers the interest of the applicant. He would receive the zoning, split and sell the lot. Criterion 4 specifically states: "The proposed zoning classification promotes the public interest and not solely the interest of the applicant". The amendment would create, proportionately more traffic on Lincoln Road, which is a minor arterial and generally not be consistent with the lot sizes on the south side of Lincoln Road Therefore,this zoning map amendment should not be approved. Planning and Zoning Commission Recommendation During the Planning and Zoning Commission meeting, a representative from District 15 and nearby property owners expressed their objections to the proposed rezoning. Bob Ungaro, School District 15, stated that Mr. Lively does not have the legal right to access the easement. The school district has concerns for the school buses and the safety of the children. In response to a question from Chairman Howell,Mr. Ungaro acknowledged he did not have advice from the school district's attorney. Suzanne Kotomski, 2619 Lincoln Road, objected to the plan to add another home to the lot. She addressed drainage issues, safety of the children, and placement of a home on the lot. Mr. Lively responded that the school district had alleviated the drainage issue when they did the improvements to the easement and that he is very aware of the presence of the school buses and children as his children attend the school. David Miller, 2717 Lincoln Road, had submitted a letter dated January 2, 2006. He expressed concern for the placement of the home, chopping the lot in two, and the potential for the driveway creating safety problems for the school. Chairman Howell read Mr. Miller's letter(attached as Exhibit A) into the record. Attorney Cahill requested time to review documents to determine if Mr. Lively had an easement that would permit a driveway. Chairman Howell called for a five-minute recess at 8:12 p.m. The hearing reconvened at 8:18 .m. with all members still in attendance. We are strongly opposed to this action for the following reasons: First, Mr. Lively should have put more thought into the positioning of his residence when it was built. He nicely placed it in the middle of the existing piece of property. If selling off a portion of this property was his intent down the road, he should have built his home centered on the western half of the property. As .positioned now, the sidewalk of the home abuts the proposed lot line. This is aesthetically unappealing. One look around the immediate neighborhood finds that the neighbor to the south is a one-plus acre parcel, and the multiple neighbors to the west are also one-plus acre parcels. Chopping this lot as proposed does not fit in with the current class of Estate. If both the city and the county put time, effort and money into the planning and zoning of properties, it should not be changed frivolously. In this case, on a whim and potential economic gain of one whose home is up for sale, an obvious sign he has no long-term interest and concern for the neighborhood. Additionally, no matter where the proposed driveway is placed, it poses safety issues for the school traffic, and congestion at best. Finally, but not conclusively, the negative economic impact on our school district far outweighs the added tax revenue it gill receive from this undesirable rezoning, As neighbors in close proximity for over 30 years, we strongly oppose this zoning change and ask that the commission deny this request. Background This property was annexed and granted C-5 Highway Commercial zoning in 1995. On May 20, 2004 the Planning and Zoning Commission approved a 2-lot subdivision for this property(Northgate East). At that time, the property was being subdivided to accommodate a single user on Lot 1, with the balance of the site to be developed in the future. The deal with the user for Lot 1 fell through and the final plat was never pursued. In 2005, to facilitate the sale of the property to the current applicant, the final plat was resurrected and brought back to the City Council for approval. Planning and Zoning Commission At their 10/6/05 meeting, the Planning and Zoning Commission unanimously recommended approval of the Preliminary and Final Plat of Subdivision for The Shops at Fox River, subject to the following conditions: 1) Revision of the plat as necessary to comply with the City's comments, 2) Approval of the site plan, landscape plan and elevations by staff prior to construction document submittal,and 3) The recordation of the plat shall occur within 6-months of City Council approval. Staff Recommendation Since receiving the recommendation for approval from the Planning and Zoning Commission, the applicant has been working diligently to address final engineering concerns. As of the date of this report, there are still some remaining unresolved items: 1) For all developments fronting a state highway, IDOT must issue a permit for proposed roadway improvements. The applicants have submitted the plans to IDOT for roadway improvements, including a 5-lane, concrete cross section extending north from Blake Boulevard to a point in the Village of Johnsburg north of the site, and a traffic signal at Diamond Drive. IDOT has performed an initial review of these improvements but has not yet issued the required permit. Staff is confident that the IDOT permit will be issued soon. 2) McHenry County has not certified the Base Flood Elevation (BFE) and not issued a watershed permit. This may cause some modification in the drainage and detention design but should not be significant enough to alter the design or layout of the overall site. Dick's Sporting Goods is anxious to break ground so that they can be open for this year's Christmas season. In order to move this project along, Staff is recommending that the preliminary and final plats be approved, with following conditions: 1) The applicant shall address all remaining engineering comments; 2) The elevations,the landscape plan and the site plan shall be approved by Staff; 3) IDOT issuing roadway improvement permit; and 4) The recordation of the plat shall occur within 6-months of City Council approval. Preliminary and Final Plat—The Shops at Fox River Northeate East Property In attendance representing the project were Chris Erb of Developer's Diversified Realty and Chris Perry and Tim Reber of Woolpert Inc. Mr.Erb, representing the real estate firm for the subject property, introduced the Preliminary and Final Plat to the Commission. Chris Perry of Woolpert detailed the project for the commissioners. This project is located north of the existing Kohl's shopping center on the east side of Route 31. Customer parking would be located on the front of the property (nearest to Route 31). A mixture of tenant uses is proposed including small box, large box and junior anchor tenants. Patrons will be able to access all commercial entities by parking in a single location. Outlot opportunities will be available for restaurants or retail tenants. Several detention areas are proposed as the property poses unique drainage challenges. The applicant has met with Illinois Department of Transportation (IDOT). A new traffic signal and Route 31 improvements are proposed as a result of the development of the subject property. A three-lot subdivision is before the commissioners for review this evening. Negotiations are ongoing with potential tenants. A lease with Dick's Sporting Goods has been signed. Elevations would include brick and stone elements. In response to an inquiry, the shopping center would be landscaped to buffer the large parking area from the entryway and highway. Pedestrian walkways will not be typical concrete but would be stamped concrete to give the appearance of brick and/or asphalt. City Planner Zeller provided the commissioners with the Staff Report regarding this matter. She noted the applicant is preliminary and final plat approval for the Shops at Fox River, a three-lot subdivision. Lot 1 is designated for restaurant or retail.Lot 3 is owned by a separate entity. Lot 2 P&Z Commission October 6, 2005 Page 2 will be the site of the multi-tenant shopping center. The signed lease tenant hopes to be open by fall 2006. Staff noted there are outstanding concerns expressed by Public Works and City Engineers. All issues will be addressed prior to Council approval and recordation. Staff recommends approval of the preliminary and final plat subject to the conditions as noted: 1. Revision of the plat as necessary to comply with the City's comments; 2. Approval of the site plan, landscape plan and elevations by Staff prior to construction document submittal; 3. Recordation of the Final Plat within six months of City Council approval. In response to an inquiry, Planner Zeller stated City Engineer's have not provided comments yet regarding the subdivision. Issues regarding drainage will be addressed upon receipt of the engineer's report. However, the applicant has been provided with comments from Community Development Department and Public Works Department. Parking would be located on each lot to support the uses contained on the site. Responding to a question, Mr. Perry stated if a building does not face Route 31, the rear or side of the building would be aesthetically pleasing to the highway. There was some concern expressed regarding the number of curb cuts along the entrance drive, as well as the ingress/egress to the subdivision. Concerns were also expressed regarding the safety of employee parking at the rear of the buildings. 1 Dumpsters would be located at the rear of the buildings and would be screened as required by ordinance. A cross access would be provided to a 19-acre commercial development to the immediate north which would be located in the Village of Johnsburg. The applicant intends to develop and market both projects as a single shopping center. Responding to an inquiry, Mr. Erb stated it is unclear at this time whether the 19-acre parcel would require an additional traffic signet to the north of the subject property. Some discussion occurred regarding the type of shopping center envisioned for the subject property and the types of tenants which would be drawn to the site. Mr. Erb gave a brief history of the evolution of Lifestyle Center type malls and shopping centers and noted their contrast to retail power centers.. Mr. Erb noted the developer of the project would be a long term owner/tax payer to the City of McHenry. In response to Chairman Howell's inquiry, there were no questions or comments from the audience. Public Hearing—City of McHenry File No.Z-653 Stade Property EIevated Water Storage Facilit-y Chairman Howell at 7:55 p.m. convened the Public Hearing for an application filed by the City of McHenry for a Zoning Map Amendment to RS-1 Low Density Single Family Residential District and a conditional use permit to allow an elevated water storage facility on the property. Chairman Howell stated notice of Public Hearing was published in the Northwest Herald on September 20, 2005. Planner Zeller stated as the City of McHenry is the applicant, she would field any questions by the commissioners. She noted the subject property is comprised of approximately one acre which would be utilized to house the City's newest municipal water tower. The property is located south of the southwest comer of the intersection of Bull Valley Road and Crystal Lake Road. The present health insurance plan covering City of McHenry employees and retirees expires March 31,2006. Renewal quotes have been received and new internal medical and dental charges have been calculated. The City Council needs to consider renewing the plan's administrative and excess insurance costs and set the plan's internal rates for employees,retirees and COBRA participants for the coming health plan year. The City's health insurance plan contains a medical,wellness(including vision), prescription card, and dental benefit provision. The wellness,prescription and dental components are entirely self-funded while the medical portion is a hybrid plan with the City responsible for self funding a primary layer of medical claims with a protective layer of excess coverage for claims exceeding the primary level. There are two types of excess coverage utilized. First,"specific"excess coverage kicks in when claims for individual participants exceed$55,000. In a typical year, only one or two claims exceed the specific layer. Second, an aggregate attachment provides an overall layer of protection for the medical portion of the plan which applies when the city's total medical claim costs meet or exceed the aggregate attachment level. For the past year,the aggregate was approximately$1,324,972. The aggregate has historically been set at 125%of the actual expected claims as determined by the insurance company's actuary. By design,the insurance company has built in enough"cushion"so that the aggregate excess coverage will only be utilized in a very poor claims year. Administrative and Excess Insurance Renewal. Annually,the City of McHenry obtains renewal quotes from the market through an insurance broker. Table A depicts the various quotes obtained. Staff recommendation is to proceed with the Standard Security with a$55,000 specific and an aggregate attachment level of$1,703,917 as highlighted in yellow. The Standard Security$55,000 specific quote represents a maximum exposure of$1,984,449 (based on 53 single and 109 family participants) or a 26.6%increase over the current year. However, fixed costs show a somewhat more modest increase of 15.4%over the current year. Internal Premium Charges. Table B shows how the internal charge for health plan participants is determined. The schedule provides a breakdown of the internal charge for fixed medical costs per participant, medical claims, vision/wellness, and prescription. In keeping all benefit levels the same as the current year and again using a specific level of $55,000, the monthly internal charge for single coverage increases from $398.09 to $581.44, single+ 1 increases from$886.70 to $1,057.28, and the family rate increases to $1,339.13 from $1,128.09. Employee premiums would remain at $0 for single employees and increase from $56.42 to $62.06 and$84.27 to $92.70 for single+1 and family coverage respectively. This represents a 10.0%increase to the employee for both rate levels, the maximum increase allowed under current collective bargaining agreements. Retirees electing to participate in the program would continue to be responsible for the entire premium. As stated earlier, the dental portion of the plan is 100% self funded with a TPA administering the program. Table C contains the recommended monthly internal dental charge of$33.18 for single coverage, $69.77 for single+ 1 coverage, and$90.39 for family coverage. Participants electing single+ 1 and family coverage are required to pay a portion of the premiums each month amounting to $17.26 and$26.19 respectively. These new amounts represent a 7.75% increase for single+ 1 and a 7.81% increase for family coverage. The entire premium is paid by the City for the single employee. Recommendations. Staff recommends the following: 1) Approval of the quote from Standard Security for a $55,000 specific as highlighted in Table A for the medical administrative and excess insurance portion of the plan; 2) Approval of the internal premium levels for medical, wellness/vision, and prescription benefits as outlined in Table B; and, 3) Approval of the internal premium levels for dental coverage as outlined in Table C.
