Special Event Liquor License Application

Issuance of a Special Event (Class D) license shall only authorize the Retail sale of Alcoholic liquor by not-for-profit organizations, for consumption on the premises during a designated special event. No such license shall be issued for a license period in excess of 3 days nor shall any private premises be eligible for more than eight such licenses during any calendar year. The fee for such license shall be $100.00 per event. The premises shall be considered unlicensed both immediately before and immediately after any Special Event license period.

Dram shop (liquor liability) insurance to the maximum limit must be obtained and a certificate of insurance naming the City of McHenry as additionally insured shall be required prior to approval of the application.

AFFIDAVIT
I hereby state that the City of McHenry shall not be held liable for any occurrence, loss, damage, or injury as a result of this event. Person or persons to whom this license has been issued shall hold the City of McHenry harmless in connection with any and all claims arising as a result of the consumption of alcoholic liquor or as a result of the issuance of this license.

The undersigned certifies that they will not violate any of the laws of the City of McHenry, of the State of Illinois, or of the United States of America, in the conduct of this Special Event Liquor License and that the statements contained in this application are true and correct to the best of his/her knowledge and belief. The undersigned is aware that state liquor licensing approval is required for this license.
