All information provided herein is true and correct and all ordinances and codes of the City of McHenry shall be complied with. I hereby represent and agree that, in considera-tion of this permit being issued, only the work herein applied for will be done and that the premises being worked on will only be used for the purposes set forth herein. I under-stand and hereby acknowledge it’s the property owner’s responsibility to ascertain if there are any existing private covenants, conditions and/or deed restrictions, which may further regulate and/or prohibit work for which this building permit from the City has been obtained. I further acknowledge it’s the property owner’s responsibility to obtain required written or other permission or follow any other private approval process from any such home, property owner’s or other association, if applicable, prior to commencing work on my property even if a building permit is issued by the City. I hereby indemnify the City, its officials and employees from any and all liability for damages, lawsuits, attorneys fees and injuries, including death sustained by anyone or damage to any property, including surveying errors and encroachment liability which accrue against the City.

Deck/Gazebo

You must also upload the following:
* One (1) copy of the plat of survey for the property. Indicate the size of the deck and the proposed location of the deck with distances from property lines, setbacks, easements and location of other structures (pools, sheds, house, etc.)
* One (1) copy of the deck building plans. Plans should include the following information:
a) Footing detail, post size and spacing
b) Beam size and spans; also joist size, spacing and spans
c) Decking material size, type of material and manufacturer specifications if not wood
d) Ledger board size and method of attachment, including type of material of the rim board of house if used for attachment
e) Distance deck floor will be above grade adjacent to deck
f) Railing detail, indicating height above deck floor, spacing of balusters, and how constructed
g) Sway bracing if required
h) Stair detail, indicating riser height and tread depth, number of steps, size of tread material, and stringer size and spacing
i) Approval from Homeowners Association. The need for approval and review procedures vary in each subdivision; contact Homeowners Association to verify requirements

Demolition

Detached Garage

* One copy of the plat of survey with the detached garage drawn on, giving dimensions away from the property lines and other structures.
* One set of construction plans for the garage with depictions and details on the concrete, gas curb, framing, size and height, etc.
* If applicable, a letter of approval from the Homeowner’s Association.

Driveway

* One (1) copy of the plat of survey for the property indicating the proposed location of the driveway or driveway expansion. Indicate the width of the driveway or driveway expansion, and indicate the proposed material for the driveway or driveway expansion, and the material of the existing driveway, if applicable.
* Homeowner Association approval if applicable.

Electric Upgrade

You must upload the following:
* One set of construction plans detailing the location of the electric box, location of the electric meter, voltage, etc.
* Copy of the electrical contractor’s up-to-date license with a stamped letter of intent.

Fence

* One (1) copy of the plat of survey for the property indicating the proposed location of the fence or dog run on the property. Use the symbol X—X—X—X—X to indicate the fence.
* Indicate the style of fence and the fence height on the plat of survey or provide a picture or photocopy of the proposed fence.
* If applicable, provide a letter of approval from Homeowner’s Association and/or letter(s) from adjoining property owner(s) indicating permission to connect to adjoining neighbor’s fence.

Pools, Hot Tubs and Spas

* One copy of the plat of survey for the property indicating the proposed location of the pool, size, height, distance from property lines, house and other accessory structures.
* Details on the type, size, and construction of the pool.

Storable Pools

* For plumbing work, we need the plumber's license registration and a letter of intent.

Sidewalks/Stairs and/or Stone/Brick Veneer

You are required to upload a copy of the construction plans.
